Giving away the out on top of not having your closer face their best hitter, stupid decisions when the pressure was on.

Watched a HS playoff series end this year when with 2 outs a guy on 2nd and down 2 runs the batter laid down a bunt and got thrown out at first.  Coaches make dumb choices.

I am old enough to remember the 1980 NLCS between the Phillies and Astros.  It was the best of 5 back then and Nolan took a 5 - 2 lead into the 8th inning of game 5 and things didn't work out.

That makes him sound even more dumb and behind the times, you knew Harper was going to come up and you were down by 1. There is no better place to use your closer than right there. That spot is the reason you traded for him, not to potentially face 5-7 if your setup man got through the top of the order.
